Enhance community health with Baycrest Ambulatory Services as a Registered Nurse (RN)!
Your role will focus on chronic disease management for seniors in Toronto.
Baycrest is looking for a dedicated RN to join their Interprofessional Primary Care Team. You will support independent seniors through health clinics and tailored wellness education, ensuring comprehensive care management. Your contributions will include developing chronic disease care plans and collaborating closely with primary care physicians and community partners to enhance care delivery.
Key Responsibilities:
• Provide expert, quality health services for clients
• Execute targeted nursing clinics and home visits
• Lead health education and promotion activities
• Support interdisciplinary care and personal interactions
• Participate in data collection and quality projects
Requirements:
• Current CNO registration and
B.Sc. Nursing Degree
• Recent experience in geriatrics and community care
• Prior knowledge of psychogeriatric practices preferred
• Reliable transportation for community visits
• Strong communication and computer skills
