The Medly Coordinator plays a vital role in patient-centered care, managing daily Medly alerts and assisting heart failure patients through education and support. Your clinical knowledge will guide medication titration, while working with various interdisciplinary teams to enhance patient outcomes and adhere to best-practice guidelines. You will be instrumental in documenting patient communications accurately in the EPIC system and ensuring adherence to regulatory standards.

Key Responsibilities:

• Manage daily Medly alerts with Nurse Practitioner and physicians

• Educate patients on heart failure self-care strategies

• Document communications in the EPIC health information system

• Collaborate with clinical teams and referring physicians

• Participate in quality improvement initiatives for optimal care

Requirements:

• Registration with the College of Nurses of Ontario

• Bachelor of Science in Nursing;
Master’s preferred

• Minimum 3 to 5 years of cardiology nursing experience

• Strong organizational skills and attention to detail

• Excellent communication abilities for patient interaction
