In this permanent position, you will collaborate with an inter-professional team and physician partners to optimize patient function and health management. As a Nurse Practitioner, you'll provide advanced care to complex inpatient cases, ensuring both medical and psychosocial needs are met. Your expertise in geriatric and general medicine will play a pivotal role in enhancing quality of care across two sites.
Key Responsibilities:
• Plan and evaluate advanced nursing care in the Reactivation Unit
• Conduct comprehensive health assessments and coordinate care
• Prescribe and monitor medication therapies
• Facilitate patient admissions and collaborate on discharges
• Lead communication on clinical and ethical issues with patients
Requirements:
• Master’s Degree in Nursing required
• Registration in the Extended Class with the College of Nurses of Ontario
• Minimum 3 years of recent experience in adult nursing
• Current Basic Life Support (BLS) certification
• Excellent communication and organizational skills
