Community Nurse Practitioner East

The Nurse Practitioner position is vital in providing primary health care within a multidisciplinary team at South Riverdale. With a focus on health education and disease prevention, you will perform health assessments, manage chronic conditions, and educate clients on lifestyle choices. This role mandates onsite work to engage effectively with the community and clients.

Key Responsibilities:

• Conduct comprehensive health assessments throughout patients' lifespans

• Manage chronic conditions and consult with physicians when needed

• Provide lifestyle counselling and supportive care

• Write prescriptions and order lab tests

• Perform home visits and coordinate community outreach
Requirements:

• Baccalaureate degree in nursing or equivalent

• Current RN(EC) registration with College of Nurses of Ontario

• 3-5 years’ community health experience

• Strong communication skills in diverse settings

• Fluency in a relevant language is a plus
