CBI Home Health is looking to hire a Primary Nurse Clinician/Supervisor in Toronto.
Salary Range:
The salary range for this position $70,000 - $90,000 annually.

What you will do:
- The Primary Nurse Clinician provides expert clinical leadership and delivers high-quality, client-centered care to individuals with complex needs in the community.
- This role combines clinical excellence, leadership, and mentorship to support sustainable care transitions, hospital avoidance, and optimized patient outcomes.
- The Primary Nurse Clinician & Supervisor, Integrated Programs will leverage an advanced nursing skill set to deliver time-sensitive, comprehensive patient visits—often within 24 hours—to stabilize complex patients, conduct thorough assessments, and develop or adjust individualized care plans.
- Deliver advanced nursing care for complex patients, including urgent, time-sensitive visits and care plan adjustments.
- Conduct comprehensive, evidence-based assessments and develop individualized care plans in collaboration with clients, families, and interdisciplinary teams.
- Mentor, support, and supervise team members to enhance clinical skills, confidence, and professional growth.
- Lead accurate, timely clinical documentation and ensure compliance with regulatory and organizational standards.
- Champion innovative, client-centered care models that support hospital avoidance, safe transitions, and continuous quality improvement.
- Develop, implement, and adjust individualized care plans in collaboration with patients, families, and interdisciplinary teams. Identify additional service needs and ensure appropriate resources are mobilized to optimize patient outcomes.
What you will need to be successful:
- Possess a current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse
- A minimum of 3-5 years of supervisory experience or equivalent clinical leadership experience in home and community healthcare.
- Advanced Clinical Expertise:
Demonstrated ability to deliver acute and advanced care in the home or community setting. - Strong Leadership
