Primary Care Nurse Practitioner

Position: Primary Care Nurse Practitioner Role
Enhance community health at Casey House as a Nurse Practitioner in primary care. This full-time position focuses on providing client-centered health solutions to underrepresented groups.

In this pivotal role, you will collaborate with Casey House and South Riverdale Community Health Centre to deliver holistic care to individuals affected by HIV and other complex health conditions. Your expertise in nursing will be instrumental in creating individualized care plans while you work collaboratively within an interprofessional team dedicated to harm reduction and health education.

Key Responsibilities:

• Perform comprehensive health assessments for diverse populations

• Prescribe treatments and adjust care plans based on client needs

• Offer guidance and crisis management to clients

• Foster collaboration with healthcare partners for seamless care

• Ensure precise documentation in Electronic Health Records

Requirements:

• Graduate degree in nursing and RN-EC license

• Minimum 3–5 years in a community health role

• Strong knowledge of mental health and substance use care

• Ability to work in a multidisciplinary team

• Valid Basic Life Support certification needed
