Join South Riverdale Community Health Centre as a Nurse Practitioner dedicated to health equity. Focus on comprehensive care, health promotion, and effective communication with patients.
In this important role, you will work independently and collaboratively to provide primary health care services that improve lives in East Toronto. Responsible for conducting health assessments and disease management, your expertise will support our mission of health promotion and community engagement for marginalized populations. This role requires onsite work to foster close relationships with clients.
Key Responsibilities:
• Perform holistic health assessments and screenings
• Consult with physicians on complex health issues
• Conduct telephone screenings and health advice
• Provide pre and post-natal care as needed
• Participate in community outreach projects
Requirements:
• Registered Nurse in Extended Class with current registration
• Baccalaureate degree in nursing from a recognized institution
• 3-5 years’ relevant experience in community settings
• Proficient in health technology applications
• Strong interpersonal skills for a multi-disciplinary approach
