Job Summary

As a member of the interprofessional care team within the Toronto Centre for Liver Disease (TCLD), the Registered Nurse (RN) provides comprehensive, patient-centered care to individuals with acute and chronic liver conditions. The Liver Clinic at Toronto General Hospital is part of one of the largest liver programs in North America, supporting patients with conditions such as hepatitis B, hepatitis C, fatty liver disease, and autoimmune liver disease, including those requiring complex, long-term management.

In this role, the RN supports a high-volume ambulatory clinic and is responsible for both direct patient care and clinic operations. Working in a fast-paced, procedural environment, the RN plays a key role in supporting clinical flow, delivering specialized treatments, collaborating with clinicians, and providing patient education to promote optimal outcomes and self-management.

Duties
  • Independently support and run assigned clinic activities within the Liver Program, ensuring safe, efficient, and patient-centered care delivery
  • Provide direct nursing care, including clinical assessment, monitoring, and ongoing support for patients with liver disease
  • Perform procedural and technical nursing skills , including: venipuncture and IV insertion; administration of albumin infusions/transfusions; independently performing paracentesis and thoracentesis procedures following appropriate training and competency validation; wound care; post-procedure recovery and monitoring
  • Monitor patient status pre- and post-procedures, recognizing and escalating clinical concerns as appropriate
  • Collaborate closely with physicians and the interprofessional team to support coordinated care and timely clinical decision-making
  • Provide patient and caregiver education related to liver disease, treatment plans, nutrition, and self-management strategies
  • Support continuity of care through follow-up, documentation, and coordination of next steps within the patient care plan
  • Maintain accurate and timely documentation within the electronic health record (Epic)
  • Contribute to a safe, organized, and efficient clinical environment, supporting clinic flow and patient throughput
  • Participate in quality improvement and practice-based initiatives aligned with program priorities
Qualifications
  • Current Certificate of Competence from the College of Nurses of Ontario (CNO)
  • Current Basic Cardiac Life Support (BCLS) certification
  • Bachelor of Science in Nursing (BScN) preferred
  • Recent experience in hepatology, gastroenterology, ambulatory care, or procedural clinic settings preferred
  • Demonstrated knowledge of chronic disease management, patient education, and care coordination
  • Strong clinical assessment, critical thinking, and decision-making skills
  • Excellent interpersonal and communication skills, with the ability to support patients with complex, long-term care needs
  • Demonstrated ability to work effectively within an interprofessional team in a fast-paced clinical environment
  • Commitment to continuous learning, professional development, and quality improvement
  • Experience with electronic health records (Epic) considered an asset
  • Demonstrated ability to meet the physical demands of a fast-paced clinical environment , including participation in patient lifting, repositioning, and transfers as part of routine care delivery
