Position: Clinical Coordinator for Heart Failure Patients
Elevate patient care as a Clinical Coordinator focusing on heart failure management through Medly. Collaborate with healthcare teams to support patient education and medication titration effectively.

This Clinical Coordinator role involves managing Medly alerts efficiently while working closely with the Heart Function Nurse Practitioner and physicians. You'll engage with patients and families to enhance understanding of heart failure management while ensuring compliance with best practice guidelines. Your documentation will be crucial in the EPIC health information system.

Key Responsibilities:

• Manage Medly alerts in collaboration with healthcare teams

• Educate patients on heart failure management strategies

• Triage alerts and coordinate communication between teams

• Document all patient interactions according to standards

• Engage in continuous improvement and quality initiatives

Requirements:

• Valid registration with the College of Nurses of Ontario

• Bachelor’s Degree in Nursing;
Master’s preferred

• 3-5 years of experience in Cardiovascular nursing

• Strong organizational and customer service skills

• Commitment to Indigenous health and cultural safety training
