As a key member of Ontario Health atHome, you will leverage your expertise in geriatrics to enhance behavioural support for patients at risk of adverse outcomes. You'll engage with interdisciplinary teams, coordinating care and conducting comprehensive assessments to create integrated care plans. Your clinical knowledge will guide patient safety and the quality of care for older adults with complex needs.
Key Responsibilities:
• Assess urgency of referrals based on patient risk factors
• Address adverse events impacting patient safety promptly
• Use standardized tools for geriatric assessments
• Collaborate with care coordinators for individualized plans
• Advocate for improved access to clinical services
Requirements:
• Master's degree in nursing, registered in Ontario
• 3-5 years in geriatrics and behavioural management
• Knowledge of Ontario Health Care delivery
• Expertise in dementia and mental health practices
• Valid CPR certificate required
