Key Responsibilities:

• Implement Chronic Disease Management interventions

• Provide clinical support within a multidisciplinary team

• Liaise with care providers for resource facilitation

• Maintain patient confidentiality and records management

Requirements:

• Bachelor of Science in Nursing or RPN diploma

• Registered with the College of Nurses of Ontario

• Minimum 2 years of clinical experience

• Proficient in Microsoft Office and EMR systems

• Fluent in English and a second language is an asset
