Interprofessional Primary Care Nurse Practitioner

As an NP, you will apply your expertise in managing acute and chronic health issues for a diverse patient population. This position focuses on assessment, treatment, and the promotion of health and wellness, addressing the needs of those without primary care.

Your role is critical in enhancing access to healthcare services and ensuring comprehensive care transitions.

Key Responsibilities:

• Conduct detailed patient assessments and formulate care plans

• Diagnose and prescribe medications within the NP scope

• Facilitate collaborative care with the interprofessional team

• Implement disease prevention and chronic disease management strategies

• Evaluate patient outcomes and support quality improvement initiatives

Requirements:

• Master’s degree in Nursing required

• Post-Master's NP diploma in Primary Health Care

• Active membership in the College of Nurses of Ontario

• 3 years of recent clinical experience preferred

• Strong problem-solving and communication abilities
