Enhance resident care as a full time Nurse Practitioner at Humber Heights. Collaborate with a multidisciplinary team to elevate health outcomes and provide expert clinical assessments.
At Schlegel Villages, this role is pivotal in delivering resident-centered care and improving health outcomes. You'll engage in comprehensive assessments while building strong relationships with both residents and their families. As a mentor and leader, you'll uphold long-term care policies and drive initiatives for clinical excellence.
Key Responsibilities:
• Deliver top-tier clinical care to residents
• Conduct and lead comprehensive geriatric assessments
• Support interdisciplinary goals in quality improvement
• Develop palliative care strategies for residents
• Participate in quality initiatives and clinical reviews
Requirements:
• Registration as a Nurse Practitioner in Ontario
• Experience in conducting geriatric assessments
• CPR certification and Current AIS Certification
• Leadership and mentoring capabilities
• Knowledge of long-term care regulations
