SE Health is hiring Full-Time and Part-Time RNs/RPNs for various locations, including Alliston and Bradford. In this role, you'll conduct comprehensive client assessments, develop personalized care plans, and collaborate with community support services. We're looking for passionate nurses who care about community health and evidence-based practices.
Key Responsibilities:
• Conduct detailed assessments and develop tailored care plans
• Act as the primary point of care for assigned clients
• Collaborate with healthcare team members for cohesive care
• Assist clients in transitioning to community support services
• Ensure continuous evaluation of client care and outcomes
Requirements:
• Registration with the College of Nurses of Ontario
• CPR certification valid upon hire
• Valid driver's license and reliable vehicle
• Willingness to work flexible shifts, including weekends
• Experience with pediatric care is a plus
