At Community Outreach Services, the Registered Nurse is integral to the Assertive Community Treatment Team. You will engage in patient support, conduct assessments, and navigate care plans using a health equity lens. Flexibility is key, as this position requires community travel and may involve weekend or on-call shifts to meet patient needs.
Key Responsibilities:
• Actively participate in the ACT team providing patient care
• Collaborate with psychiatrists, social workers, and peer support
• Conduct biopsychosocial assessments and treatment planning
• Respond to patient safety concerns and crises
• Adapt to operational needs across mental health services
Requirements:
• Registered Nurse with BScN and current Ontario license
• Minimum 2 years of clinical experience in mental health
• Competent in physical health assessments and medical nursing
• Proven skills in crisis prevention and management
• Experience with diverse communities and health equity practices
Utilize your nursing expertise and mental health training to support individuals in need at Michael Garron Hospital.
