REGISTERED PRACTICAL NURSE (RPN) – Apotex 7

Baycrest Hospital and the Apotex Jewish Home for the Aged provide specialized geriatric care for older adults with complex medical, cognitive and dementia-related needs. Apotex 7 is a Cognitive Support Unit. Residents on this floor have a wide range of cognitive and physical changes, from mild to severe cognitive impairment and from independently ambulating to wheelchair bound.

• Collaborate with the Registered Nurse to provide sound clinical knowledge, act as a clinical expert and resource, and provide comprehensive, quality client/family centred health care services from admission through transfers and discharge.
• Plan for, deliver and evaluate nursing interventions, including medications and treatments within the RPN’s scope of practice.
• Provide client‑centred care within an interdisciplinary team to meet personal care, activation, recreation and spiritual needs.
• Perform client assessments.
• Collaborate with the Registered Nurse to develop and revise individualized care plans.

• Current registration in valuable standing with the College of Nurses of Ontario (CNO).
• Current Basic Cardiac Life Support (BCLS) certificate, Level C Automated External Device (AED).
• Experience in long‑term care, clients in day programs or residents in a long‑term care and community setting (preferred).
• Psychogeriatric experience; mental health assessment and intervention and working with responsive behaviours in various types of dementia, delirium, and mental health issues (preferred).
• Certificate in Dementia Care (preferred).
• Training in Gentle Persuasive Approach and U‑First (preferred).
• Excellent interpersonal, communication and problem‑solving skills.
• Excellent verbal and written communication skills.
• Basic computer skills, including MDS/RAI, Point Click Care, Microsoft Word and Excel (preferred).
