Sherbourne Health is seeking committed Registered Nurses for its Family Health Team. You'll provide comprehensive care, conduct health assessments, and lead primary care appointments. The role emphasizes collaboration with clients and families while supporting community health initiatives, including vaccination clinics and health education.
Key Responsibilities:
• Provide comprehensive nursing care for diverse clients
• Conduct health assessments and triage urgent health needs
• Implement preventive health and health restoration interventions
• Develop client care plans with multidisciplinary teams
• Facilitate referrals to internal and external health services
Requirements:
• Current Registration with the College of Nurses of Ontario
• Baccalaureate degree in nursing or diploma entry RN
• 3 to 5 years nursing experience in community health
• Basic Life Support certification
• Experience with priority populations, particularly 2
SLGBTQ communities
