In this role, you will support a 391-bed long-term care home from 1:00 p.m. to 8:00 p.m., working 26 hours bi-weekly, including weekends. Candidates should bring at least one year of geriatric or long-term care experience and a current Nurse Practitioner registration in Ontario. You will enhance staff competencies and provide specialized resident care through leadership and expert assessments.
Key Responsibilities:
• Oversee interdisciplinary team care initiatives
• Mentor nursing staff on clinical practices
• Deliver comprehensive care to residents
• Conduct educational programs for family and staff
• Engage with community partners for continuity of care
Requirements:
• One year experience in geriatric or long-term care
• Registration with the College of Nurses of Ontario
• Familiarity with Long-Term Care Act and related legislation
• Ability to pass a Police Vulnerable Sector Check
• Italian language skills are an asset
