Reporting to the Director of Care, the Registered Practical Nurse plays a critical role in modifying care plans and monitoring health indicators for residents. This position emphasizes collaboration with families and healthcare teams to ensure personalized care delivery. Your leadership will inspire and empower other team members while fostering continuous improvement in care standards.
Key Responsibilities:
• Modify care plans based on residents' health status
• Maintain relations with residents, families, and team members
• Supervise and direct PSW activities on the unit
• Conduct unit rounds and administer medications
• Document assessments and maintain accurate care records
Requirements:
• RPN status with the College of Nurses of Ontario
• Familiarity with RAI-MDS is a plus
• Well-developed communication skills
• Strong relationship-building capabilities
• Recent TB test and clear vulnerable sector screen
