As an Advanced Practice Nursing Clinician in the Toronto Central region, you will significantly impact the lives of older adults facing responsive behaviours. This hybrid role requires a master's degree in nursing, with a strong emphasis on assessment and intervention in geriatrics and mental health. Utilize your expertise to triage referrals and develop integrated care plans that address the unique needs of patients in the community.

Key Responsibilities:

• Assess referral urgency based on patient risk factors

• Address adverse events affecting patient safety

• Use standardized assessment tools for older adults

• Conduct in-home assessments focusing on responsive behaviours

• Collaborate with interdisciplinary teams for care planning

Requirements:

• Master’s degree in nursing with registration in good standing

• 3-5 years of related experience in geriatrics

• Knowledge of the Ontario Health Care system

• Competence in geriatric mental health practice

• Valid CPR/Basic Cardiac Life Support certificate
