SE Health is seeking compassionate Registered Practical Nurses committed to community care. You will conduct comprehensive client assessments, develop personalized care plans, and ensure excellent care delivery in coordination with the care team.
Your role will involve supporting clients through care transitions and connecting them with necessary community support services.
Key Responsibilities:
• Conduct client assessments and develop tailored care plans
• Lead primary care accountability for assigned clients
• Collaborate with care teams on client care decisions
• Support clients with local community resources
• Ensure continuous evaluation and improvement of care strategies
Requirements:
• Current registration with the College of Nurses of Ontario
• CPR certification and a valid driver's license required
• Canadian Nurses Association specialty certification preferred
• Ability to meet physical demands including lifting and movement
• Must have clear criminal and immunization records
