The University Health Network is seeking a Registered Nurse specialized in wound care to enhance patient outcomes in non-traditional settings. This role addresses complex social determinants affecting those with the highest health needs. You'll work collaboratively with an interdisciplinary team to implement innovative care models that directly benefit community members.
Key Responsibilities:
• Deliver primary care in various community settings
• Provide wound care and chronic disease management
• Conduct outreach with the Mobile Health Clinic team
• Advocate for health equity and access to services
• Collaborate with healthcare professionals for coordinated care
Requirements:
• Current CNO registration as a Registered Nurse
• 3+ years clinical experience, ideally in community health
• Expertise in chronic wound management and assessment
• Strong skills in engaging marginalized populations
• Valid Ontario Driver’s License required
