Join Scarborough Health Network as a Full-Time Nurse Practitioner, specializing in primary care and chronic disease management. Work collaboratively to enhance community health services and support diverse populations.
In this dynamic role at the Centenary Site, you will play a pivotal part in providing health care to individuals lacking primary care coverage. Your responsibilities will include conducting assessments, devising care plans, and advocating for patients through effective interdisciplinary teamwork. Use your skills to champion health promotion and chronic disease management, fostering well-being in the community.
Key Responsibilities:
• Assess patient needs through detailed evaluations
• Develop and manage coordinated health plans
• Diagnose and prescribe treatments as per NP standards
• Enable psycho-social support and coordination services
• Establish links for specialist referrals and care
Requirements:
• Master's degree in Nursing; NP Diploma required
• Valid registration with the College of Nurses of Ontario
• Minimum of 3 years clinical experience as a Nurse Practitioner
• Knowledge of community health practices
• Strong collaborative and communication skills
