Baycrest Hospital has an exciting opportunity for a REGISTERED PRACTICAL NURSE (RPN) in the Complex Continuing Care Unit (6 East).

Unit/Program: Complex Continuing Care Unit – older adults with multiple chronic health conditions in a non-acute hospital setting.

Location: Baycrest Hospital

Position Type: Permanent Part-Time

Shift Type: Day-Evening, Alternate Weekends (subject to change)

Bi-Weekly

Hours:

30 Hours, 7.5 hrs/shift

Union: SEIU Service Part-Time

Hourly Pay Rate: $38.41 - $39.02

At Baycrest Hospital, older adults receive specialized post-acute inpatient care informed by deep expertise in aging and brain health, delivered with compassion and clinical excellence. The Complex Continuing Care program provides ongoing in-hospital care to improve quality of life for medically complex patients.

The Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The program helps patients and families identify strategies that support quality-of-life goals as well as short-term and long-term care needs.

• Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments within the RPN scope of practice
• Provides client-centred care within an interdisciplinary care team to meet personal care, activation, recreation and spiritual needs of clients
• Collaborates with the Registered Nurse to develop and revise the individualized care plan

• Current registration, in good standing, with the College of Nurses of Ontario (CNO) is required
• Current Basic Cardiac Life Support (BCLS) Level C certificate;
  Automated External Defibrillator (AED) required
• Experience in geriatrics with complex medical care needs preferred
• Psychogeriatric experience (mental health assessment and intervention) and working with responsive behaviours preferred
• Excellent interpersonal, communication and problem-solving skills
• Basic computer skills – Meditech, Microsoft Word, and Excel preferred
