Baycrest Hospital has an exciting opportunity for a REGISTERED PRACTICAL NURSE (RPN) in the Complex Continuing Care Unit (6 East).
Unit/Program: Complex Continuing Care Unit – older adults with multiple chronic health conditions in a non-acute hospital setting.

Hourly Pay Rate: $38.41 - $39.02

The Complex Continuing Care program provides ongoing in-hospital care to improve quality of life for medically complex patients.
Job SummaryThe Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The program helps patients and families identify strategies that support quality-of-life goals as well as short-term and long-term care needs.
Key Responsibilities- Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments within the RPN scope of practice
- Provides client-centred care within an interdisciplinary care team to meet personal care, activation, recreation and spiritual needs of clients
- Collaborates with the Registered Nurse to develop and revise the individualized care plan
- Current registration, in good standing, with the College of Nurses of Ontario (CNO) is required
- Current Basic Cardiac Life Support (BCLS) Level C certificate;
Automated External Defibrillator (AED) required - Experience in geriatrics with complex medical care needs preferred
- Psychogeriatric experience (mental health assessment and intervention) and working with responsive behaviours preferred
- Excellent interpersonal, communication and problem-solving skills
- Basic computer skills – Meditech, Microsoft Word, and Excel preferred
