This permanent full-time role entails coordinating home dialysis for patients, ensuring their education and support in independent care. You will be integrally involved with a multidisciplinary team, improving patient outcomes through collaboration and personalized support.
Key Responsibilities:
• Assess and educate patients and families on home therapies
• Coordinate and facilitate patient appointments and interventions
• Collaborate with the interdisciplinary team for comprehensive care
• Monitor quality indicators for treatment adequacy
• Participate in rotational on-call support and training
Requirements:
• Registered with the College of Nurses of Ontario
• Minimum 2 years of nephrology experience
• Previous home hemodialysis experience preferred
• Strong patient assessment and education skills
• Valid driver’s license and access to a vehicle
