Key Responsibilities:

• Conduct patient assessments, diagnoses, and treatments

• Refer clients to specialists or community resources

• Facilitate collaborative case conferences for service coordination

• Maintain standards of care per nursing regulations

• Document client encounters accurately in the EMR system
Requirements:

• Current registration as an Extended Class RN

• Experience in primary care nursing in community settings

• Ability to work with diverse and non-insured populations

• Knowledge of infection control practices

• Strong communication and organizational skills
