Join Humber River Health as a part-time Registered Nurse in the Home Hemodialysis Program, combining patient care with education and support. This role emphasizes flexibility and a commitment to patient outcomes while providing care at home and in the community.
This permanent part-time position is responsible for providing comprehensive patient care for hemodialysis. You will engage in care paths, monitor safety standards, and facilitate appointments collaboratively. With a focus on professional development, you'll contribute to quality improvement initiatives and assist patients and their families through transitional care.
Key Responsibilities:
• Deliver patient education and care in various settings
• Coordinate patient appointments with clerical teams
• Collaborate with interdisciplinary teams for patient support
• Conduct community and home visits for patient care
• Document clinical data accurately in Meditech
Requirements:
• Registered in good standing with the College of Nurses of Ontario
• Minimum 2 years nephrology experience required
• BLS Provider certification needed
• Proficient in Microsoft Office and Meditech
• Valid driver's license and access to a personal vehicle
