As part of the Nephrology Program, your primary responsibilities will revolve around providing comprehensive care for hemodialysis patients. You'll utilize a self-management approach to empower patients, ensuring they are active participants in their own care journey. Quality assurance activities will also be part of your role, ensuring high standards of patient care and operational efficiency.
Key Responsibilities:
• Provide hemodialysis care and education to patients
• Participate in quality assurance activities for care delivery
• Cover the Transition Care Unit as needed
• Maintain patient records and conduct assessments
• Collaborate with the healthcare team to enhance patient outcomes
Requirements:
• Graduate from a recognized Practical Nursing program
• Current RPN registration with the College of Nurses of Ontario
• Minimum one year acute care experience required
• Knowledge of hemodialysis procedures preferred
• Effective communication and assessment skills
