Join LOFT Community Services as a Registered Nurse in Toronto, Ontario, focusing on transitional care for older adults. This onsite, full-time role demands strong clinical skills and leadership.
As part of a dedicated team at The Path Home – Pine Villa, you’ll engage in comprehensive nursing assessments and deliver customized care to enhance the lives of older adults facing health challenges.
Your role includes supervising RPNs, training PSWs, and coordinating nursing activities to ensure optimal support for residents in need. Help facilitate a safe environment where clients can thrive in their recovery journey.
Key Responsibilities:
• Develop and implement individualized care plans
• Support RPNs in delegated nursing acts
• Lead medication management and ensure compliance
• Provide mentorship and training to PSWs
• Engage with community partners for coordinated care
Requirements:
• Registration with College of Nurses of Ontario
• Bachelor’s degree in Nursing required
• At least 3 years of relevant nursing experience
• Proven ability to work with complex mental health needs
• Excellent communication and mentoring skills
