Join Sherbourne Health as a Full-Time Registered Nurse to make a difference in the community. This role highlights primary nursing care and leadership in public health initiatives.
As a key member of the Sherbourne Health Family Health Team, you will focus on providing essential nursing care. Responsibilities include conducting health assessments, leading immunization clinics, and facilitating client referrals. Your expertise will support community initiatives targeting marginalized populations, including specialized health programs.
Key Responsibilities:
• Conduct health assessments, history taking, and triage
• Lead primary care appointments for chronic disease management
• Deliver health education responsive to client needs
• Collaborate on client care plans with multidisciplinary teams
• Implement community health initiatives, including vaccination clinics
Requirements:
• Registered Nurse with the College of Nurses of Ontario
• Nursing degree or diploma with BLS certification
• 3 to 5 years of community health experience
• Familiarity with healthcare equity for marginalized populations
• Strong interpersonal and organizational skills
