Position: Part-Time Nurse for Family Health Team

Carefirst FHT is looking for a Part-time Registered Nurse to join its compassionate and collaborative team. The role involves coordinating care interventions and engaging in community health promotion activities. Candidates should be experienced in chronic disease management and possess strong interpersonal skills to facilitate care across various settings.

Key Responsibilities:

• Coordinate outreach and health education interventions

• Support multidisciplinary care and collaborate with team members

• Conduct community health promotion activities

• Facilitate referrals within the Family Health Team

• Manage clinical supplies and patient files

Requirements:

• RN or RPN from an accredited institution

• Registered and in good standing with Ontario’s nursing college

• 2+ years in community or primary healthcare

• Exceptional customer service and telephone skills

• Flexible schedule including evenings and weekends
