Carefirst is expanding its dynamic Family Health Team and is seeking a Registered Nurse. This role encompasses key responsibilities in patient education, health management, and teamwork in a collaborative setting. Ideal candidates should excel in a fast-paced environment and have experience in chronic disease management to support community health initiatives effectively.

Key Responsibilities:

• Implement care management for chronic diseases

• Deliver clinical support in a team-based care model

• Facilitate health education sessions and community outreach

• Ensure compliance with nursing standards and practices

• Maintain patient records, ensuring confidentiality

Requirements:

• Bachelor of Science in Nursing or equivalent diploma

• Valid registration with the College of Nurses of Ontario

• Minimum of two years clinical experience required

• Strong attention to detail and multitasking ability

• Fluent in English, additional languages an asset
