In this role, you will work collaboratively within a multi-disciplinary team to provide comprehensive primary care services to registered clients. Your efforts in assessment, treatment, and health education will address individual and community health needs. The position requires adherence to nursing regulations and delivering coordinated care ensuring the best outcomes for patients.
Key Responsibilities:
• Perform comprehensive patient assessments and health education
• Refer clients to other health services as necessary
• Participate in inter-provider case conferences for service continuity
• Uphold nursing standards and policies within practice
• Ensure patient records are promptly updated in the EMR
Requirements:
• Registered Nurse in the Extended Class with Ontario
• Primary care experience in diverse settings
• Experience serving marginalized populations
• Familiarity with infection control procedures
• A commitment to community health initiatives
