You'll work collaboratively with physicians, nursing leaders, and an interdisciplinary team to deliver resident‑centred care, champion clinical excellence, and drive quality improvement initiatives across multiple long‑term care homes.

What You'll Do

As the Nurse Practitioner, you will be a clinical leader responsible for providing advanced nursing care while supporting the ongoing development of best practices across the organization.

Clinical Practice
  • Provide comprehensive assessments, diagnoses, treatment planning, and ongoing management of residents.
  • Practice autonomously within the Nurse Practitioner scope while collaborating with physicians and the interdisciplinary team.
  • Develop, implement, and evaluate individualized resident care plans.
  • Order, interpret, and follow up on diagnostic testing.
  • Prescribe, adjust, or discontinue pharmacological and non‑pharmacological therapies.
  • Coordinate referrals, consultations, virtual care, and specialist services.
  • Support smooth transitions for residents returning from hospital.
  • Champion palliative and end‑of‑life care.
  • Advocate for residents and families while ensuring exceptional quality of care.
Leadership
  • Serve as a key member of the Centre Management Team.
  • Partner with nursing leadership to support staff performance and probationary reviews.
  • Lead quality improvement initiatives and clinical best practice implementation.
  • Participate in policy, procedure, medical directive, and protocol development.
  • Act as a clinical resource for complex resident care situations.
  • Promote evidence‑informed decision making throughout the organization.
  • Mentor and coach Registered Nurses, Registered Practical Nurses, Personal Support Workers, and students.
  • Identify educational opportunities and deliver clinical teaching.
  • Support staff in integrating current research and best practices into daily care.
  • Develop educational resources for residents, families, and caregivers.
Research & Quality Improvement
  • Participate in clinical research and knowledge translation.
  • Evaluate resident outcomes and identify opportunities for continuous improvement.
  • Contribute to presentations, publications, and professional practice initiatives.
  • Support organizational quality management projects and nursing excellence.
What You Bring

Required Qualifications
  • Current registration with the College of Nurses of Ontario (CNO) as a Nurse Practitioner (Extended Class - Primary Health Care or Adult).
  • Master's Degree in Nursing (MScN).
  • Demonstrated experience practicing as a Nurse Practitioner or extensive experience in geriatric or long‑term care settings.
  • Strong clinical assessment and diagnostic reasoning skills.
  • Exceptional communication and relationship‑building abilities.
  • Excellent conflict resolution and critical thinking skills.
  • Strong organizational and time management skills.
  • Proficiency with Microsoft Office and Point Click Care  (or similar electronic health record systems).
  • Knowledge of current legislation, best practices, and standards related to Long‑Term Care.
  • Ability to maintain confidentiality and exercise sound professional judgment.
Preferred Qualifications
  • Experience in Long‑Term Care or geriatrics.
  • Gerontological Nursing certification.
  • Palliative and End‑of‑Life Care education or certification.
  • GPA, PIECES, or other seniors‑focused education.
  • Fluency in Chinese is considered a strong asset.
