Registered Nurse at UHN's Social Medicine

We are seeking a passionate Registered Nurse (RN) to join the Gattuso Centre for Social Medicine and Population Health. This full-time position involves providing primary and episodic care to high-need patients in community settings. You will work closely with an interdisciplinary team to enhance the quality of care for marginalized populations while utilizing expertise in wound care and chronic disease management.

Key Responsibilities:

• Deliver primary care in non-traditional settings

• Assess and respond independently to patient needs

• Support outreach services with the Mobile Health Clinic

• Advocate for health equity and access to care

• Collaborate with multidisciplinary teams for continuity of care

Requirements:

• Registered Nurse with CNO, BScN preferred

• 3+ years of clinical experience, especially in community health

• Expertise in chronic wound care and clinical assessments

• Strong communication and relationship-building skills

• Valid Ontario driver’s license for patient visits
