As a Clinical Coach, you'll collaborate with Ontario Health and local organizations to enhance palliative care competencies.
Your role includes providing coaching and support to healthcare professionals and engaging with Indigenous organizations to ensure inclusive practices. You'll participate in regional planning and contribute to the Palliative Care Health Services Delivery Framework effectively.
Key Responsibilities:
• Provide coaching to improve palliative care competencies
• Collaborate on local service integration for quality care
• Consult on complex clinical care needs for patients
• Support Indigenous communities in implementation efforts
• Attend regional meetings to aid in planning
Requirements:
• Master’s degree in Nursing required
• Minimum five years of palliative care experience
• Registration with the College of Nurses of Ontario
• Expertise in quality improvement initiatives
• Strong clinical coaching and consultation skills
Utilize your expertise in palliative care and strong communication skills to support community organizations across Ontario.
