UHN is recruiting a Registered Nurse dedicated to addressing health inequities in community settings. This role emphasizes compassionate care for patients with complex medical challenges, utilizing interdisciplinary collaboration to improve health outcomes. You'll actively contribute to client navigation and education while supporting marginalized communities, including those facing homelessness and mental health issues.
Key Responsibilities:
• Provide episodic and primary care in non-traditional settings
• Assess and manage chronic wounds and other health conditions
• Participate in Mobile Health Clinic outreach initiatives
• Advocate for systemic changes to improve access to care
• Engage with various professionals to ensure continuity of care
Requirements:
• Registration in good standing with CNO as an RN
• 3+ years of experience in relevant clinical areas
• Proven expertise in managing wound care effectively
• Comfortable working with underserved and diverse populations
• Ability to travel across Toronto with valid driver's license
