As an integral part of our team at Humber Heights, you will provide exceptional care to our residents while collaborating with another Nurse Practitioner.
Your role requires experience in geriatric assessments, mentoring healthcare professionals, and maintaining CPR certification. The position emphasizes quality improvement and a resident-centered approach within a compassionate healthcare culture.
Key Responsibilities:
• Conduct comprehensive geriatric assessments for residents
• Support resident-centered palliative care and holistic services
• Collaborate with interdisciplinary teams on quality initiatives
• Guide assessments, diagnostic testing, and procedural communication
• Foster relationships with residents, families, and staff
Requirements:
• Current Nurse Practitioner registration with the College of Nurses of Ontario
• Expertise in completing geriatric assessments
• Heart Save Level CPR certification required
• Prior experience in mentoring healthcare teams
• Knowledge of Long-Term Care Homes Act and relevant legislation
