Position: Toronto Care Supervisor (RN/RPN) Needed
Join CBI Home Health as a Care Supervisor (RN/RPN) in Toronto, where you will lead care initiatives across multiple community sites. Your primary focus will be on patient safety and staff development.

In this float position, your leadership will support teams in delivering exceptional home health care. You will perform comprehensive assessments, document care plans, and mentor field staff, ensuring that patient and provider standards are upheld. Your proactive management will drive community health delivery initiatives in a dynamic environment.

Key Responsibilities:

• Respond quickly to patient referrals and needs

• Conduct thorough assessments and maintain clear documentation

• Evaluate risks to ensure both patient and provider safety

• Coach and support field staff to enhance performance

• Ensure all services meet CBI Home Health's quality standards

Requirements:

• Current registration with the College of Nurses of Ontario

• Minimum 2 years of supervisory experience in community services

• Proven ability to establish rapport with clients and staff

• Valid Ontario driver's license and reliable transportation

• Ability to find solutions within policies and best practices
