Make a difference in patient lives as a Registered Nurse in the Home Hemodialysis Program at Humber River Health. Lead home therapy initiatives and provide crucial support and education.
As part of Humber River Health's innovative team, you will manage case activities and ensure patients are trained in home dialysis practices. This full-time role emphasizes collaboration with a multidisciplinary team to deliver exceptional care. Participate in initiatives that embody patient-centered decision-making and contribute to chronic disease management.
Key Responsibilities:
• Perform patient assessments and home environment evaluations
• Provide training for independent dialysis care
• Engage and educate families on treatment processes
• Collaborate on quality improvement initiatives
• Collect and document patient data accurately
Requirements:
• Valid registration with the College of Nurses of Ontario
• 2 years of recent nephrology experience required
• Previous experience in home dialysis is advantageous
• Proficiency in Microsoft Office and Meditech
• Strong communication and organizational skills
