Registered Nurse Position

Position: Humber River Health Registered Nurse Position
Take on a rewarding part-time role as a Registered Nurse in the Home Hemodialysis Program at Humber River Health. This position requires a strong focus on patient education, direct care, and the ability to work collaboratively in a home setting.

As part of the Nephrology Program, you will play a key role in patient care, monitoring quality indicators, and enhancing safety in hemodialysis. Your responsibilities include providing assessments, coordinating appointments, and engaging in continuous professional development to ensure the highest standard of care.

Key Responsibilities:

• Perform home visits to ensure patient care adequacy

• Provide education and training related to independent dialysis

• Facilitate patient appointments in collaboration with the clerical team

• Support CQI initiatives and clinical practice improvements

• Collect and document clinical data for reporting

Requirements:

• Valid registration with the College of Nurses of Ontario

• At least 2 years nephrology and hemodialysis experience

• Certification as a BLS Provider

• Ability to utilize Microsoft Office and Meditech systems

• Driver's license and access to a personal vehicle required
