Pine Villa seeks a dynamic Nurse Practitioner to deliver exceptional care for adult and senior populations. This position offers a unique chance to work with a collaborative interprofessional team in a transitional setting.
As a Nurse Practitioner at Pine Villa, you will focus on caring for clients with complex needs after hospital admission. Located at Eglinton and Bathurst, this site emphasizes teamwork with professionals from Sunnybrook Health Sciences Centre and other community partners. Your primary role will be to assess, diagnose, and implement comprehensive care plans.
Key Responsibilities:
• Develop and execute individualized patient care strategies
• Collaborate on interprofessional care plans
• Utilize advanced decision-making in clinical scenarios
• Serve as a resource in strategic planning and problem-solving
• Foster community partnerships for continual care
Requirements:
• Ontario Nurse Practitioner (Extended Class) registration required
• Master’s in Nursing or equivalent expertise required
• Minimum 3 years of relevant patient care experience
• Experience in geriatric and mental health care
• Strong analytical skills and CPR certification required
