Position: Full-Time Nurse Practitioner at Humber River Health
Become a transformative nurse practitioner with Humber River Health in North West Toronto. This full-time role focuses on clinical care and collaboration within the Nurse Led Outreach Team.

As a Nurse Practitioner, you will lead clinical care in long-term care settings while working closely with multidisciplinary teams. Your expertise will facilitate hospital discharges and minimize emergency department visits, ensuring continuity of care. This role demands strong analytical skills and collaboration with various healthcare stakeholders to enhance patient outcomes.

Key Responsibilities:

• Provide comprehensive assessments and care within Nurse Practitioner scope

• Collaborate with hospital and LTC teams for discharge planning

• Build capacity in LTC staff for acute treatment protocols

• Integrate best practices and evidence-based guidelines

• Address medication reconciliation upon discharge

Requirements:

• Master in Nursing or Nurse Practitioner qualification

• 1-3 years of Nurse Practitioner experience required

• CNO registration in Extended Class as an NP

• Strong communication and interpersonal skills

• Valid Ontario driver’s license required
