In this crucial role within the Nephrology Program, you will provide all necessary care for hemodialysis patients. You will educate patients, engage in self-management models, and collaborate closely with interdisciplinary teams for seamless patient journeys. Quality assurance is essential in delivering top-tier care and improving patient access to services.
Key Responsibilities:
• Deliver comprehensive care for hemodialysis patients
• Provide education and follow-up for effective care management
• Engage in quality assurance activities for service improvement
• Manage patient documentation and health assessments
• Potentially cover other units as assigned
Requirements:
• Qualified RPN from an accredited program
• Active registration with the College of Nurses of Ontario
• One to two years of acute care experience required
• Preferred experience in hemodialysis
• Strong judgment, empathy, and communication skills
