In this role, you will coordinate clinical activities and work directly with patients using a case-managed approach. Your expertise will be crucial in providing education, training, and support for independent home hemodialysis, ensuring quality and safety in treatment.
Key Responsibilities:
• Perform patient assessments and home visits
• Educate patients about dialysis processes
• Monitor treatment quality and patient safety
• Facilitate appointments in collaboration with team members
• Act as a liaison with the interdisciplinary team
Requirements:
• Active registration with the College of Nurses of Ontario
• Minimum 2 years of nephrology experience required
• Proficient in data collection and documentation
• Strong communication and interpersonal skills
• Commitment to ongoing professional development
