Join Humber River Health as a Registered Nurse specializing in Home Hemodialysis. Lead patient care through education and clinical coordination in home settings.
This permanent full-time position involves supporting chronic kidney disease patients in their journey to independent home dialysis. You will use your clinical expertise to educate and assess patients, performing home visits while coordinating with healthcare team members for comprehensive support.
Your role is crucial in ensuring the safety and adequacy of home dialysis treatments.
Key Responsibilities:
• Provide patient assessments and education on dialysis
• Conduct annual recertification and monitoring of quality indicators
• Support patients during their transition to home therapies
• Schedule and facilitate patient appointments collaboratively
• Engage collaboratively with the interdisciplinary healthcare team
Requirements:
• Registered with the College of Nurses of Ontario
• 2 years nephrology experience; 18 months in hemodialysis
• BLS certification required; CNeph certification preferred
• Proficient in Microsoft Office and Meditech
• Excellent communication and organizational skills
