Key Responsibilities:

• Conduct patient assessments and develop care plans

• Collaborate on patient care strategies with the care team

• Provide effective teaching and health education

• Evaluate care plans and document progress

• Facilitate seamless transitions and referrals as needed

Requirements:

• Active General Class Registration with College of Nurses of Ontario

• BLS Certification required

• Surgical experience within last 3 years mandatory

• Experience in patient education an asset

• Proven skills in planning and coordination
