This Registered Nurse position is vital in delivering comprehensive assessments, interventions, and educational support to patients and their families. Your recent experience in General Medicine nursing will be key in providing high-quality care within the Integrated Community Program. Setting priorities in a fast-paced setting while demonstrating commitment to quality improvement is essential.
Key Responsibilities:
• Conduct thorough assessments and plan patient interventions
• Work seamlessly with healthcare teams for integrated care
• Educate patients on health management and discharge plans
• Facilitate a smooth discharge process for patients
• Prioritize tasks effectively to enhance care
Requirements:
• Current registration in good standing with the College of Nurses of Ontario
• Minimum two years experience in General Medicine
•
B.Sc.N. preferred, along with Medical-Surgical certification
• Strong interpersonal and communication skills
• Dedication to patient-centered care initiatives
