SE Health is on the lookout for dedicated Registered Practical Nurses to carry out client assessments, craft personalized care plans, and implement evidence-based practices. As a primary care provider, you will coordinate with team members to facilitate smooth care transitions and engage clients with their local support services, ensuring they receive the best care possible.
Key Responsibilities:
• Conduct detailed client assessments and devise care plans
• Lead care for assigned clients and collaborate with teams
• Assist clients in transitioning to community support services
• Evaluate care plans based on client needs and best practices
• Facilitate communication among care team members for optimal outcomes
Requirements:
• Current College of Nurses of Ontario registration
• CPR certification required upon hire
• Valid driver’s license for required travel
• Passion for community health and excellence in care
• Capability to meet physical demands of nursing
