Nurse Practitioner - Nurse LED Outreach Team; BIR

Position Overview

Reporting to the Clinical Manager of the Nurse‑Led Outreach Team (NLOT), the Nurse Practitioner (NP) provides clinical care for residents in Long‑Term Care (LTC) homes within a sub‑regional boundary. The NP collaborates with physicians, coordinators and other members of the inter‑professional team to reduce avoidable Emergency Department visits, prevent hospitalizations and facilitate safe transitions to and from hospital settings.

• Practice in accordance with the Regulated Health Professions Act, Gerontological Nurses Association (GNA) Standards, the Long‑Term Care Homes Act, the Practice Standards set by the College of Nurses of Ontario and the standards set for the Nurse Practitioner.
• Assist in preventing hospital admissions for current LTC residents with new or increasingly complex medical or equipment needs not accommodated in their LTC home.
• Support placement of eligible high‑needs designated Alternate Level of Care (ALC) patients from hospitals into LTC.
• Facilitate admission of community members with specialized needs to LTC homes.
• Mitigate non‑urgent transfers to the region’s Emergency Departments.
• Provide capacity building for LTC staff to enhance competency with acute treatment protocols.
• Conduct assessment, intervention, monitoring and follow‑up for acute and chronic illnesses.
• Lead and promote excellence in patient‑centred nursing care through expert practice, education, consultation and research.
• Participate actively in a Regional NLOT Community of Practice.
• Contribute to development and implementation of integrated hospital‑community‑LTC clinical pathways.
• Develop linkages with external partners to ensure continuity of care.
• Resolve medication reconciliation issues upon discharge from hospital.
• Utilize the Clinical Information System.
• Utilize protective equipment and report hazards.

• Master’s Degree in Nursing/Nurse Practitioner (MN‑NP) or Master of Science in Nursing with successful completion of a post‑Masters Acute Care Nurse Practitioner Certificate Program from a recognised school of graduate studies.
• Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care).
• Completion of a CNO‑approved controlled substances education.
• Professional Liability Protection (RNAO or equivalent).
• Current Basic Cardiac Life Support (BCLS) certification.
• Three years of demonstrated experience as a Nurse Practitioner.
• Previous experience in the LTC sector (preferred).
• Additional specialty education in Gerontology (preferred).
• Three to five years of Acute relevant medicine or Primary Care experience (preferred).
• Strong communication and interpersonal skills, ability to collaborate with physicians, inter‑professional team members and stakeholders.
• Ability to maintain and protect patient confidentiality with discretion and sound judgment.
• Commitment to research and quality improvement activities related to patient care across the continuum.
• Commitment to the mission and values of SHN.
• Consistent and regular attendance.
• Valid Ontario Driver’s license and access to a car.
• Clinical expertise: IV initiation, suturing, venipuncture, wound care, end‑of‑life care planning.
• Clinical leadership to coordinate patient care with caregivers.
• Self‑starter, ability to work independently and collaboratively, time‑management skills, advanced nursing assessment, and therapeutic knowledge.
• Excellent organisational, interpersonal, communication and critical appraisal skills.
• Superior written and oral communication skills.

• Ability to speak another language.

• Medical‑Surgical training and education.
• Gentle Persuasive Approach course.
